High stress corporate environments can lead to high blood pressure, poor dietary habits and sleep routines, and an office or travel-based lifestyle with insufficient regular exercise. In addition, a man in his late 30s, or 40s and 50s enters a prolonged phase of male menopause. This comes with falling testosterone levels, poor glucose control and a pre-diabetic state, weight gain especially around the middle, a softer erection and silent cardiovascular disease. In fact, erectile dysfunction is caused by circulation problems in over 60-70% of men and can pre-date a heart attack by two and a half years.

Who is this screen for?

If you are a man with a busy corporate lifestyle, and find weight coming on year on year, have been or are a smoker or have a family history of heart disease or find that your nighttime erection (morning wood) is softer and less frequent than before or feel you are not quite as potent as you once were – then this screening program is for you.

What tests does this screen contain?

Nurse assessment

For body composition measurements, BP, risk factor and medication record.

Urine test

For blood cells, sugar and protein.

Blood tests

  • Full blood count – low haemoglobin levels affect the ability of the circulation to carry oxygen; High white cell counts can signify infection which is common in diabetics.
  • Glycated haemoglobin or HbA1C – a measure of average blood sugar control over 3 months
  • Kidney function – high blood pressure and diabetes can cause kidney failure from a variety of circulation-related mechanisms.
  • Liver function – silent diabetes and poor dietary habits can cause a fatty liver.
  • Lipid (cholesterol) profile – high cholesterol, or co-existing poor thyroid function and pre-diabetes, can cause blockages in the arteries to the heart, brain, legs and the penis.
  • hs-CRP – The high-sensitivity C-reactive protein (hs-CRP) test is a protein that measures general levels of inflammation in your body. The hs-CRP can be used to find the risk for heart disease and stroke in people who don’t already have heart or arterial disease.
  • Comprehensive Male hormone profile, with Total, free and bioavailable Testosterone, Sex hormone binding globulin, Oestrogen, Prolactin, Thyroxine, Cortisol (the stress hormone), Follicle stimulating hormone (FSH) and Interstitial cell stimulating hormone (ICSH) – these hormones interact with each other to create the andropause, affecting the circulation and the erection in the ageing male. A one-off testosterone test alone does not reveal the full picture.
Ankle and toe pressures and Ankle-brachial index

Measures drop in pressure in the feet compared to the arm, that can indicate critical decreases in blood flow to the legs and blockages in leg arteries.

CT- Calcium score

Identifies potential blockages in the heart arteries and the risk of having a heart attack.

Treadmill test

To look for exercise-induced ischemia (decreased blood flow) in the heart, indicating undiagnosed heart disease and risk of a heart attack.

Carotid intima media thickness

This is measured by ultrasound and is an overall measure of the risk of having an adverse cardiovascular event.

Consultation that includes a SHIM scoring (Sexual Health Inventory for Men) with a senior specialist vascular surgeon

To discuss results, identify severity of erectile dysfunction and the need for further testing, assess the state of heart and circulation health and make recommendations to reverse any issues identified.

CT angiograms of the heart or legs are NOT included in our package, as they involve the use of radiological contrast with exposure to the very small, but nevertheless present, risk of contrast allergy and kidney toxicity. In addition, the heart rate will need to be artificially lowered with medication for the CT coronary angiogram. These are recommended by us only if needed for identified disease or if potential procedures are planned and not as part of routine screening.

How do I prepare for this screen and how long does it take?

  • If you have booked this screen, you should have received instructions on what to expect on-email or phone. If not, please call the clinic at least 1 day before the screen on +65 64720503 during working hours to confirm the instructions.
  • The screen is conducted over 3 days, scheduled as per your convenience. Plan about 5 hours in the clinic on the first day, a 1-hour trip (plus travel time) to the CT scan on the second day and about 1.5 hours on the third day.
  • You will need to come in the morning on Day 1 having fasted (without having had anything to eat) for at least 6 hours. You may have clear non-sugary drinks, black coffee or black tea.
  • Please DO NOT take your diabetic medication, if prescribed for you, on the morning of Day 1, but you should bring the medication with you. You must, however, take any other medication you take for blood pressure or blood thinners etc with water as normal.
  • Come prepared for a brisk walk on the treadmill. Bring along a water bottle, t-shirt and shorts or gym attire, and running shoes on Day 1.
  • Avoid smoking or caffeinated drinks for 2 hours or so before the CT Calcium score scan on Day 2.

What can I expect on the screening days?

  • On Day 1 of the screen, you will first be registered for the screening.
  • You will then be asked to provide a mid-stream urine sample whilst still fasting.
  • Following the urine sample, you will be asked to rest in a quiet comfortable environment for about 30 minutes (for more accurate sex hormone and stress hormone assessments) and then have your blood tests.
  • You may now go and have breakfast and return to clinic where you will have the nurse assessment and can have your diabetic medication, if any, as well.
  • You may now change for all remaining tests including the treadmill test to be done on Day 1. Day 1 may need 4-5 hours to complete.
  • On Day 2 (which can be scheduled to your convenience) is the day you have your CT Calcium score scan of the heart at a radiology centre (not in our clinic). The test itself takes about 30 minutes but do factor in travel time to and from the radiology centre.
  • Day 3 (which can be scheduled to your convenience but would be at least 3-5 days after Day 2 to allow for all test results to come back in), you will see the specialist vascular surgeon for a consultation and discussion. The SHIM scoring for severity of any erectile dysfunction is done during this consultation in privacy with the vascular surgeon. Day 3 may need up to 1 hour to complete. Please eat and take medications as normal on Day 3.

What if my screening detects a problem?

  • We will give you a detailed report and recommendations in writing to take away.
  • If screening results suggest you need further investigations or treatment, you will also be given a note for you to show your family doctor to make a formal referral to us or to a cardiologist if needed. Further treatments or tests may not be covered by insurance if there is not a formal referral.
  • Conditions related to sexual function are often not covered by insurers. If any further investigations for this are needed, we are happy to proceed after discussing it with you.
  • Your screening results and reports are NOT shared by us with your doctor or any third party including your insurers or employers. Sharing your report results with other doctors, insurers or employers is for you to do as you deem fit but is recommended.

How often is an Executive Men’s Health Circulation Screen needed?

We recommend that you have this comprehensive screen every 2 years or so unless new symptoms develop, to ensure that your circulation stays healthy, and any progression of disease is detected early.
