What Is Vascular Erectile Dysfunction?

Vascular erectile dysfunction (ED) is a specific subtype of erectile dysfunction characterised by inadequate blood flow from blockages in the arteries leading into the penis, or excessively rapid outflow of blood from the veins out of the penis, preventing the maintenance of an erection suitable for sexual performance.

In over 60-70% of patients, ED is the result of an underlying circulation problem affecting the arteries or veins making vascular disease the commonest cause..

Why Choose Minimally Invasive Procedures?

Choosing minimally invasive procedures for the treatment of vascular erectile dysfunction offers several advantages over traditional surgical methods.

  • Reduced Risk of Complications: Using smaller incisions or needle punctures in minimally invasive procedures reduces the risk of infection and bleeding, making these options safer for the patient.
  • Shorter Recovery Times: These procedures often result in quicker recovery, allowing patients to return to their daily activities and work more rapidly.
  • Less Pain and Discomfort:: Patients experience less post-operative pain and discomfort following minimally invasive procedures. This reduces the need for pain relief medications and their potential side effects during recovery.
  • Improved Cosmetic Outcomes:: With smaller incisions or, in some cases, no incisions, minimally invasive procedures tend to leave minimal scarring.

Types of Minimally Invasive Procedures

These are the various minimally invasive procedures available for treating vascular erectile dysfunction, each with its unique mechanism of action in addressing the underlying vascular issues contributing to ED:

Intracavernosal Injections

Intracavernosal Injections involve the direct injection of vasodilator medications into the penis. These medications help to relax the smooth muscle within the penile arteries, promoting increased blood flow and facilitating an erection.

For men with vascular ED, intracavernosal injections can provide a quick and effective way to achieve erections, especially in cases where oral medications are ineffective. However, these are often not preferred by patients as they have to inject themselves, and do not feel spontaneous.

Endovascular Arterial Revascularization with angioplasty

Arterial revascularization angioplasty is a minimally invasive procedure aimed at improving blood flow to the penis in men with specific blockages in the inflow arteries to the pelvis and penis – usually the internal iliac and internal pudendal arteries. It is typically reserved for men who have proven blockages usually from cholesterol deposits in the arteries that supply blood to the penis causing ED.

Arterial revascularization with a targeted balloon angioplasty opens up blood flow in the blocked or narrowed arteries, restoring blood flow to the penis and facilitating natural erections.

Venous Leak Surgery

Venous Leak Surgery addresses erectile dysfunction caused by venous leak, a condition where the veins of the penis fail to adequately restrict blood flow out of the penis during an erection. This is usually seen in younger men in their 30s or 40s.

In the past, this surgery involved tying off the leaking veins to prevent the premature loss of the erection. It was a relatively major procedure that was considered when conservative treatments had failed and if a clear venous leak has been identified through diagnostic testing. The procedure had a high failure rate of over 50% within 6 months of the procedure.

The minimally invasive Glue ablation for venogenic impotence (GAVI) procedure is a much smaller procedure that takes 30 minutes or so under a short general anaesthetic. It involves injecting medical glue under direct vision and x-ray guidance into the leaking vein at the base of the penis to slow down the venous leakage. The procedure has higher success rates and minimal downtime, and is a day surgery procedure.

Post-Procedure Care and Management

Following the completion of any minimally invasive procedure for vascular erectile dysfunction, post-procedure care and management ensure optimal recovery and effectiveness of the treatment.

  • Immediate Post-Procedure Care
Immediately after a procedure, patients may receive specific instructions on wound care, medication to manage pain or prevent infection, and guidance on physical activities.
  • Long-Term Follow-Up
Regular follow-up appointments allow medical specialists to assess the efficacy of the treatment, make necessary adjustments, and monitor for any long-term complications.
  • Managing Side Effects
Each procedure carries its own set of potential side effects, and managing these effectively is a key component of post-procedure care. For example, patients using vacuum erection devices might experience penile bruising or numbness, which usually resolves with adjusted use.
  • Lifestyle Modifications and Support
In addition to medical treatment, lifestyle modifications can improve erectile function and overall cardiovascular health. Patients are often encouraged to adopt a healthy diet, engage in regular physical activity, quit smoking, and manage stress well.

Frequently Asked Questions (FAQs)

Does insurance cover minimally invasive procedures for ED?

Coverage varies by insurance provider and policy. Many insurers cover diagnostic procedures like penile Doppler Ultrasound but may have different policies for treatment procedures. It’s important to consult with your insurance company for specific coverage details.

How long do the effects of intracavernosal injections last?

The effects of intracavernosal injections can last between 30 minutes to an hour, depending on the medication used and the person’s response.

Is arterial revascularization a permanent solution for ED?

Arterial revascularization can provide a long-term solution for ED in some patients, especially those with erectile dysfunction due to pelvic or perineal trauma. The success and permanence of the outcome depend on various factors, including the patient’s overall health and adherence to post-operative care.

Are there any age restrictions for undergoing minimally invasive ED treatments?

Minimally invasive ED treatments are generally suitable for adult men of all ages. The suitability of a specific procedure depends on the person’s overall health, the cause of ED, and other factors.
